DRH Workflow for Inpatient COVID Vaccination

COVID-19 vaccination is now available for administration to inpatients at DRH, including first and second doses of vaccine, per patient or provider request.  Ordering providers should perform a risk assessment to ensure clinical stability of their patients prior to receiving vaccine and request vaccine type based on the patient and/or their preference as well as based on first dose when applicable.  Vaccination should be deferred in patients who have abnormal vital signs, worsened oxygenation status, or acute mental status or functional status changes relative to baseline, active COVID-19 infection, or who have received COVID-19 monoclonal antibody treatment within 90 days.
 
At this time, COVID-19 vaccine administration to DRH inpatients will be coordinated by members of the Clinical Nurse Specialist team with the bedside RN. DRH providers should follow this workflow (link to flow diagram) Monday to Friday by placing a medication message for vaccine administration. We ask that providers initiate this process as soon as appropriate during the patient’s admission so that arrangements can be made for administration prior to the day of discharge.

This is a tip sheet for using Maestro Care to designate a visitor for an inpatient using a FYI flag. A new Designated Visitor FYI flag will be available on Maestro Care starting 6/24/2020 to support the new visitor policy of allowing one designated visitor per inpatient. The flag can be added by inpatient nurses, case managers, inpatient and ED Financial Care Counselors and operations administrators. COVID-19 Vaccines for Homebound Patients

COVID-19 vaccines for homebound patients residing in Durham County are now available through a partnership between the Durham County Community Paramedics and the Durham County Health Department. This initiative serves to identify patients who are unable to safely access the COVID-19 vaccine outside of their home due to health or mobility, and provide the vaccine to patients in their home. Referrals for Duke patients can be e-mailed to Elissa Nickolopoulos, MSW, LCSW-A at elissa.rumer@duke.edu.  All referrals must contain patient's name, MRN, preferred phone number, allergies, preferred phone number and alternative contacts (if applicable). Documentation of patient's verbal consent for referral to Community Paramedics must be documented in patient's chart prior to making the referral.
