Beginning Wednesday, June 16, 2021, the escalation process for critical lab values within the Ambulatory Clinic setting will be enhanced to include providers and tiered escalation plans. Currently, notifications of critical lab values vary by clinic and personnel, resulting in inconsistent responses to alerts. The new lab escalation process is a streamlined pathway and is direct to the provider. The average volume of critical labs per ambulatory clinic is less than 10 per month. According to the Centers for Disease Control and Prevention (CDC), more than 6.8 million doses of the Johnson & Johnson (Janssen) vaccine have been administered in the U.S. as of April 12. The CDC and Food and Drug Administration (FDA) in a joint statement on Tuesday, April 13, recommended a pause in the use of Johnson & Johnson's (J&J's) single-dose COVID-19 vaccine out of an abundance of caution. Criteria for Discontinuation of Special Airborne Contact Isolation

This document provides updated guidance to healthcare workers on when to discontinue special airborne contact (SAC) isolation for patients with COVID-19 (Discontinuing isolation). This guidance applies to all areas of Duke Health including adult and children’s inpatient, outpatient, surgical, procedural and other care areas. CDC now recommends using a symptom-based strategy alone for discontinuation of special airborne contact isolation for patients infected with COVID. Enough evidence has accumulated to show that patients are not shedding live, infectious virus after a specified amount of time passes from symptom onset. As such, Duke Health will no longer use the test-based strategy (i.e. 2 negative tests) to discontinue Special Airborne Contact isolation for patients with COVID and begin using the symptom-based strategy outlined below. Guidelines for the Use of Different Models of N95 Respirators at Duke Health

N95 respirators (or PAPRs) continue to be required for direct care of patients on Airborne (e.g., tuberculosis, measles) or Special Airborne Contact (e.g., suspected or confirmed COVID infection) precautions. Additionally, the CDC recommends that healthcare workers in areas where there is moderate to substantial community transmission of COVID-19 use N95 respirators when performing procedures that are high-risk for generating aerosols, since healthcare workers in facilities with moderate to substantial community transmission are more likely to encounter asymptomatic or pre-symptomatic patients who may be COVID-19 positive. This can include patients who tested negative during their pre-procedure test, but then develop symptoms in subsequent days. To accommodate these recommendations, Duke has made additions to the recommended PPE for staff in perioperative and procedural areas where high-risk aerosol-generating procedures are performed. This document has been updated to include two additional types of N95 respirators. 

 

Additionally, to ensure availability of N95 respirators when needed, DUHS has deployed new models of N95s to certain care areas. Staff should use the N95 respirator model that is available at their location and per instructions of their local leadership. We have answered several frequently asked questions and highlighted key features of the different types of N95 respirators in the table below. Updated 05.28.21.

Inpatient Visitor Restriction Update in Spanish – Overnight Visitor

We realize that loved ones are a vital part of a patient’s care team and play an important role in recovery. As we have navigated each phase of the COVID-19 pandemic over the last year, the safety of our patients, visitors and team members has been our highest priority. Given the decreased community spread of COVID-19 and increased vaccination rates, we will modify our inpatient visitor restrictions to permit for one overnight visitor. ED & INPATIENT Hospital Day Less Than or Equal To 3 Decision Tree

This document shows a decision tree for COVID-19 testing for patients who are in the ED or who have been admitted for less than or equal to 3 days. The decision for COVID-19 testing should be based on clinical judgment. Prior to testing for COVID-19, the Infection Prevention team recommends ruling out other potential causes of the patient's clinical condition including influenza or other non-influenza viral illnesses (as applicable). If the aforementioned tests are negative or if the clinical picture is not explained by a non-influenza pathogen, it is appropriate to proceed with COVID-19 testing. Updated 04.17.20

Criteria for Adequate Eye Protection

Duke Health recommends the use of eye protection for all staff having direct interaction with patients and visitors. Direct interaction is defined as face-to-face interaction such as entering a patient’s care environment (e.g. exam room, hospital room, bay, etc.) or coming within 6 feet for any duration of time. Face shields are the preferred eye protection as they provide full coverage of the eyes and protect the underlying face mask from potential contamination.

We recognize that some team members are seeking alternatives to face shields. Supply chain issues have made purchase of large quantities of safety goggles challenging. As such, this resource is meant to provide guidance for selecting alternate adequate eye protection to the widely available Duke-approved face shields. Uploaded 09.25.20
