Manager Resource - Instructions for Accessing COVID-19 Vaccination Reports

Thank you for your support of our ongoing effort to enhance the safety of our workplace and community through the vaccination of our providers and staff against the COVID-19 virus. All team members and affiliates are eligible to be vaccinated. Although the COVID vaccine is not a current condition of employment, it is highly encouraged. As a manager, you should know and monitor the vaccination status of the team members in your area of responsibility through the Occupational and Environmental Safety Office reporting function. Using these instructions, please plan to run the reports most useful to you on a regular basis.  

For team members with no vaccination indicated, please plan to explore the reasons for the missing information during a one-on-one conversation. Should questions arise that you do not feel comfortable responding to, feel free to invite the team member to submit the questions to EOHWCovidVax. We suspect that many of our team members have been vaccinated at a location other than a Duke vaccination clinic. If this is the case, please ask the team member to submit documentation of vaccination to the Duke VaxTrax web form. Uploaded 5.28.21
